Ageing Well Clinic

This is a pilot clinic at present, running once a week from Portland Community Hospital and due to start in January 2022

Patients with a variety of different issues including frailty, poor mobility, falls risk and risk of recurrent hospital admissions will be invited to this clinic for a holistic review of their needs. Eventually this may be extended to include patients with chronic respiratory conditions and diabetes.

The aims of the clinic include improving overall wellbeing, maintaining independence, reducing falls risk, optimization of medication and reducing risk of hospital admission.

Individuals invited to clinic will move through a series of stations so they can see a variety of health and care professionals including :

Therapist (Occupational therapist and/or physiotherapist)


Specialist Frailty Nurse / Nurse Practitioner

Specialist Respiratory or diabetes Nurse 

Social Prescriber

Health Care Assistant

Care Co-ordinator

Individuals invited to clinic are asked to complete a questionnaire before attending, and to bring this with them to the clinic. They will need to attend for about 2.5 hours

This pilot clinic is running with the full support of the GP surgeries in Weymouth and Portland. The intention is that it will be rolled out to Weymouth too later in the year.
